The Primary Care Health Home (PCHH) program is a healthcare initiative funded by MO HealthNet to offer comprehensive care management services to Medicaid participants who have qualifying chronic conditions. This initiative strives to provide full-circle care coordination and care management services as well as address social determinants of health, for patients enrolled in the program.
Your personal team of healthcare providers will include your Primary Care Provider, Nurse Care Managers, Care Coordinators and Behavioral Health Consultants who work together for YOU. Enrollment is FREE – no out of pocket costs to program participants!
Our team is here to provide you with patient-centered healthcare and offer additional resources for our patients enrolled to help them maintain and reach their personal and healthcare goals. Whether that be assisting you with referrals to specialists, helping to schedule/arrange provider appointments, ensuring that you are getting routine labs and diagnostic imaging completed on time, assisting with outside community resources, or refilling your medications in a timely manner, we are here to help and serve you so that you are meeting all of your healthcare needs!
